Concealed/open carry proponents say we are safer with such people around. How are we supposed to know the armed person we see actually has a permit? Could it not confuse and desensitize us to true threats?

First, unless they are not paying attention, the only people you will know are carrying a gun are those who are carrying openly. You will never know a person who is carrying a concealed firearm has one. And this makes up the huge majority of those who carry.
Secondly, is the person acting violent or crazy? Are they doing anything illegal? Or are they minding their own business and harming no one? If so, what reason do you have to be afraid, just because they have a gun?
More people will be killed by drunken drivers this year than in shootings. So how do you tell if the driver next to you on the highway is drunk and a danger to you or others? By his behavior. Is he driving crazily? Get away from him and call the police. If not, then go on about your business. The same logic should apply to people with guns.
